Forms
Each form is available in both a .pdf and .doc version. Use the .pdf version for viewing the forms on-line and the .doc version for saving the form to your computer. If you wish to complete an adoption application electronically, please SAVE the .doc version to your computer (right-click and select save Target as..) before entering any information. After filling out the application, please ensure you save the file on your computer. The completed form can then be e-mailed to main@wssh.org.
